  1. Download and complete an Adult Health History or Pediatric Health History, or if you are a returning client, a Patient Progress Report in both events the Check-In form if you are a returning patient. Send it to us. You may print the forms after inputting information electronically then printing, or printing first then filling the form out by hand. You may use Email, Mail, or Fax.
  2. Using the telephone number(s) you gave us on the Health History, we will call you to schedule your Initial or Follow Up appointment. Once we have your email address and the appointment scheduled, the Wyndgate Health electronic medical record system (EMR) will send you an appointment reminder. See Contact Us.
  3. After you have your appointment, log onto Quest Diagnostics laboratory web site, select a convenient draw site location (the New Brighton site is closest to Wyndgate Health) and make an online appointment for your blood and urine specimen collection (using “Routine Lab Test”). NOTE that this Quest Diagnostics appointment must be a time after your Wyndgate Health visit and allow for drive times.

    After pressing Find, Quest Diagnostics will send you an appointment reminder if you enter your email address. Allow for a one hour appointment at Wyndgate Health followed by appropriate drive time to get to your Quest Diagnostics appointment on time. In most cases, there is no need for fasting before the samples are taken. We will tell you if needed. The Quest Diagnostics scheduling tool includes Google’s door-to-door mapping and driving directions. Keep the Wyndgate Health phone number handy if you are not sure of the directions.

    The New Brighton draw site is nearest, followed by Minneapolis (Nicollet Mall), or Edina draw sites.
  4. If you are currently taking any zinc supplements, remember to stop taking it at least 24, and no more than 36 hours before your draw site appointment.
  5. At Initial or Follow Up appointments you will receive a routine physical exam, detailed review of your health history document, discussion of your symptoms and we will listen to your goals for change. We will develop an initial recommendation, order lab tests specifically focused on your condition. Wyndgate Health will give you a Quest Diagnostics lab test requisition at the end of your appointment. Don’t forget to take the requisition to the Quest Diagnostics office you selected.
  6. Foundational supplements are very often recommended at the end of your appointment. They may be purchased at Wyndgate Health so you can begin your program as soon as possible. Most supplements sold at Wyndgate Health are priced from 15 to 25% below suggested retail.
  7. Schedule your Wyndgate Health Results Consultation before or while checking out at your initial or follow up appointment.
  8. Travel to the Quest Diagnostics Patient Service Center that you scheduled above. Give the receptionist the request form given to you by Wyndgate Health. Quest Diagnostics will submit their testing fees to your health care insurance carrier. (As a separate entity, Wyndgate Health does not bill insurances of any type and expects payment in full at the time of service.)
  9. At your Results Consultation appointment, we will review our findings with you and finalize your prescription and dietary recommendations developed for you.
  10. Once finalized, Wyndgate Health will give you your final separates prescription (in the case of starting treatment with separates or not using compounding), or send it to one of our compounding pharmacy partners. Otherwise, the compounding pharmacy will ship your program to you when it is finished.
