Getting Started... Willmar, MN Clients
The Willmar, MN location operates as a satellite office to the St. Paul office. All communication, scheduling and case management is operated from St. Paul. Intake interviews and physical exams are conducted at the Willmar, MN location.
- To begin, come to this website and, from the Downloads & Resources page, download the Health History (adult, follow-up client or pediatric versions as appropriate) and the Appointment Check In form by double clicking on them one at time. Print both following the instructions at the top of the page.
- When you call, send both completed forms to Wyndgate Health at the address below.
- When Wyndgate Health receives them, they will call you to arrange an appointment once when we have the forms.
- Appointments are held on Monday evenings in Willmar, MN at 1520 Willmar Ave. SE in the Hands on Therapy office. (THANKS FOR NOT CONTACTING THE HANDS ON THERAPY OFFICE OR STAFF regarding your Wyndgate appointment.)
